To register as a new client, please complete the form below including your name, date of birth, what you are hoping for in your counseling experience, who referred you to Life's Journey Counseling, and if you will be using insurance or not. If you are using insurance, please indicate which insurance carrier you have.

New Client Form

Once your information is received you will be registered in our online Electronic Health Record, Therapy Appointment. You will receive an email invite to complete your registration forms as well as demographic and billing information. Please note, this expires 48 hours after hitting your inbox.
